Library Card Application
All patrons must fill out an official application form.
A library card is free to all residents of Cannon County. Please bring valid identification with picture ID, your name, and current address.
A phone number that we can reach you at is also required. The library card application is available at the library’s front desk or you
may download and complete the application from the link: Adams Memorial Library Card Application and return it in person to the Library.
Library cards are available to all Cannon County residents
Requirements and Guidelines
- Adams Memorial Library cards are free for all Cannon County residents.
Requirements for a Library Card:
- Patrons must have a valid photo ID with current address, or;
- Valid photo ID and an acceptable piece of mail with current address (utility bill, bank statement, lease or rental agreement, or other type of bill)
- Children, aged 5 to 17, must have a parent or legal guardian present when applying for a library card. Parents must sign the agreement and provide proof of address.
Guidelines for a Library Card:
- Replacement cards are $2.00 per replacement.
- A valid library card is required in order to check out library materials.
- A library card is not needed to use public computers.
